Abstract
66歳男．5年程前から左前頭部に腫瘤を自覚していた．2013年より，右舌癌化学療法後のPET-CTにて左前頭部皮下にfluorodeoxyglucose（以下，FDG）の軽度取り込みを認めており，2015年5月のPET-CTにてstandardized uptake valueの上昇を認めた．皮膚生検施行されたが，組織型が舌癌の扁平上皮癌とは異なるため，皮膚原発の腫瘍として当科に紹介された．病理組織学的に真皮から皮下組織，帽状腱膜にかけて好塩基性の腫瘍細胞が大小の管腔構造，篩状構造の集簇を形成しており，神経周囲への浸潤を認めた．以上の所見より，primary cutaneous adenoid cystic carcinomaと診断した．術後5か月経過し，再発所見を認めていない．本疾患は神経周囲に浸潤することが多く，局所再発をきたしやすいため，広範囲切除や長期間の経過観察を要する．
