Abstract

Significant premaxillary retrusion requires significant augmentation for correction. The technique described allows the surgeon to custom sculpt intraoperatively each implant to fit the needs of the individual patient. In cleft situations, the implant described may be carved in an asymetric contour to fit the asymmetry of the defect. The Proplast-HA implant allows for solid fixation to the underlying tissues and bony framework. The design of the implant also allows for accurate midline positioning in relation to the nasal spine. This implant allows for advancement of not only the nasolabial area, but the entire naso-alar base as well. We have encountered minimal problems with extrusion and infection in our current large series of patients. We recommend the procedure as a safe, simple, and effective means of significant premaxillary augmentation. Significant premaxillary retrusion requires significant augmentation for correction.
