Abstract
Objective: Arterial hypertension (AH) after acute ischemical stroke (AIS) significantly worsens the prognosis in such category of patients. Left ventricle (LV) velocity and time indices abnormalities by tissue Doppler imaging (TDI) as a sign of LV remodeling are associated with heart failure development and is a sensitive marker of adverse outcome. The objective of the study was evaluation of mitral annulus motion indices in the patients with hypertension after AIS. Design and method: We studied 30 patients (pts) with AH after AIS (mean age 62,5 ± 10,1 years). All patients received conventional treatment: angiotensin II receptor blockers (ARBs), calcium channel blockers and thiazide diuretics. Control group included 30 AH pts without AIS. All pts underwent EchoCG with pulsed wave (PW) TDI of mitral annulus motion evaluation. Regional contractility abnormalities was a criterium for exclusion from the study. TDI velocity and time indices of mitral annulus septal segment were studied. Results: We can see in the pts with AH after AIS PW TDI revealed significant worsening of systolic LV myocardial function in comparing with pts with AH without AIS such as ejection fraction 56,2 ± 8,3 in comparing 62,9 ± 4,2 in pts without AIS and diastolic LV myocardial function such as acceleration time 85 ± 4,47 in comparing 61 ± 2,53; deceleration time 123,9 ± 3,46 in comparing 112,4 ± 3,80; IVR time 92,3 ± 4,57 in comparing 82,3 ± 5,6. All dates are statistically significant. Conclusions: In the pts with AH after AIS PW TDI revealed significant worsening of systolic and diastolic LV myocardial function compared to those without AIS, which suggests the necessity of more ’aggressive’ blood pressure in this specific group of pts.
