Abstract

Experience of various types of extracranial-intracranial (EC-IC) bypass for the posterior circulation is presented; subtemporal superficial temporal artery-supracerebellar artery (STA-SCA) bypass,18 paramedian suboccipital occipital artery-posterior inferior cerebellar artery OA-PICA bypass,10 and occipital artery-posterior cerebral artery OA-PCA bypass, and/or OA-SCA bypass via the paramedian suboccipital supracerebellar transtentorial (SCTT) approach8 and other bypasses including saphenous vein interposition graft.9 The author have experienced 45 surgeries of these kinds on 44 patients, while around 500 EC/IC bypasses for the anterior circulation have been performed in the same period. The series includes cases of eight intractable aneurysms and two of moyamoya angiopathy besides most cases of vertebrobasilar occlusive disease. Indication of such surgeries and their results along with technical consideration by representing typical cases are described and discussed.
