Abstract

ABSTRACT The diagnosis and treatment of posterior shoulder instability presents a significant challenge to the treating orthopedic surgeon. Although a significant number of patients may show symptomatic improvement following directed physical rehabilitation, many will continue to suffer from disabling posterior instability. A review of the historical perspective, pertinent anatomy and biomechanics, clinical presentation, and the principles for surgical intervention is presented. Published reports have proven that both open and arthroscopic techniques can be effective. The primary focus of this article is to present the relative indications and surgical technique of an open posterior capsulorrhaphy through an infraspinatus-splitting approach.
