Abstract

Statement of the Problem: We present a series of cases and proposed indications for the use of a modified rigid external distraction device as an adjunct to panfacial fractures treatment where open reduction and internal fixation (ORIF) is compromised. Rigid external distraction devices, primarily used in distraction osteogenesis for correction of facial deformities from clefting or hypoplasia, have been reported for successful management of patients limited to maxillofacial fractures. Rehrmann reported the use of a head cap and symphyseal pin for management of bilateral condylar fractures, but no published reports exist that illustrate the select cases when a rigid external distraction device is a useful adjunct in panfacial fractures. This is also the first report of the combined use of a rigid external distraction device and mandibular external fixation pins in primary panfacial fracture repair where preservation of anteroposterior and vertical support is necessary.
