Abstract

Setting: An acute freestanding inpatient rehabilitation hospital with an associated network of subacute programs and outpatient facilities. Program: A well-established continuum of care. Program Description: A continuum of care was established to provide rehabilitation services in the most cost effective manner to achieve the maximal functional gain. The patients were assessed either by a nurse liaison or a physiatrist prior to admission to acute or subacute rehabilitation. The levels of care available were acute inpatient rehabilitation–freestanding hospital, subacute in a freestanding skilled nursing facility, and outpatient therapy—either comprehensive day rehabilitation or single service.
