Abstract

Setting: Tertiary care hospital. Patient: A 19-year-old man with low back pain. Case Description: The patient complained of acute onset low back pain. He presented to the emergency room with “burning,” 10 out of 10, constant, right-sided low back pain that radiated to his right calf and worsened with hip flexion. The symptoms began after mountain hiking with an 18-kg (40-lb) backpack. He denied constitutional symptoms. Radiographic studies were negative. A diagnosis of muscle strain was made. Despite 3 weeks of nonsteroidal anti-inflammatory drugs and muscle relaxants his symptoms worsened.
