Abstract

EuroQoL EQ-5D-5L is a self-reported instrument used to value health status. The objective of this study was to describe and compare EQ-5D-5L population norms in France, Germany and Spain. Data on EQ-5D-5L and EQ-VAS from the National Health and Wellness Survey (NHWS) were available for France, Germany and Spain (2018). We applied the same methodology as previously done for Japan and China; problems reported by EQ-5D-5L dimension, published index values and EQ-VAS scores were analyzed by country. The sample was weighted to be representative of each country on age and sex, and then standardized for comparison between countries. In total, 15,000 respondents participated in each of France and Germany, and 7,000 in Spain (mean age±SD 48.8±16.9 /50.1±16.5 /47.5±15.5 years and 44.3%/49.9%/48.8% men, respectively). In the three countries, problems in the pain/discomfort dimension were the most frequent. Differences between males and females were observed for the five dimensions in France (p<0.01); for all dimensions except mobility in Germany (p<0.01) and for the usual activities, pain/discomfort and anxiety/depression dimensions in Spain (p<0.001). Women reported worse scores than men except for self-care in France and Germany. Mean value index and mean EQ-VAS were 0.905 and 73.4 in France, 0.855 and 74.3 in Germany and 0.865 and 74.7 in Spain. Women reported significantly lower value index and EQ-VAS scores than men (p<0.001). After standardization, proportion of respondents reporting no problems differed significantly between countries for almost all dimensions (p<0.001), EQ-5D-5L index values were different with France having higher values followed by Spain. Inversely, EQ-VAS scores in France were significantly lower than in Germany and Spain (p<0.001). Mean EQ-5D-5L index values and EQ-VAS scores were different for France and consistent for Germany and Spain. The results varied between countries after controlling for demographic confounders.
