Abstract

Abstract—The clinical presentation of pemphigus vulgaris as localized cutaneous lesions without mucous involvement is a rare event. We report a case of a 37-year-old man who presented an inflammatory plaque on her back studded with pustules. A histopathologic study showed an eosinophilic spongiosis pattern and a direct immunofluorescence test revealed IgG deposits between the epidermal keratinocytes. Treatment with dapsone was started and cutaneous lesions cleared within three weeks. The patient remains free of lesions after several years of follow-up.Eosinophilic spongiosis is a histopathologic pattern that may be frequently found in early stages of pemphigus, but in addition, this pattern is a marker for a peculiar clinical and histological presentation of pemphigus vulgaris.
