Abstract

We report 3 cases of Pneumocystic carinii pneumonia (PCP) in patients with biopsy-proven Wegener's granulomatosis and rapidly progressive glomerulonephritis. These opportunist infections occured at the acute phase of immunosuppressive therapy including high dose corticosteroids and cyclophosphamide. Risk factors for infection are discussed. Appropriate adjustment of the cyclophosphamide dosage to the severity of initial renal impairment, usefulness of neutrophils and lymphocytes counts monitoring and systematic use of aerosolized pentamidine isethionate for PCP prophylaxis are emphasized.
