Abstract
Trapped lungとは遠隔期の炎症に起因する線維性被膜で臓側胸膜が覆われ肺拡張が制限された状態である．患者は23歳，男性．検診で左気胸を指摘され当院外来を受診した．胸部X線写真，CTでは左肺は完全に虚脱していたが，自覚症状が無いため気胸の発症時期は不明であった．前年の検診時の胸部X線写真で気胸はなく，左肺尖にブラを認めた．胸腔ドレナージ後のCTでは肺の拡張は不十分であり，臓側胸膜の肥厚を認めた．肺拡張が得られず空気漏れも停止しないため手術を施行した．左肺上葉に破綻したブラを認めブラ切除を行った．左肺は白色調の厚く硬い被膜で覆われ拡張不良であり，trapped lungと診断した．肺剥皮術を行い肺拡張の改善が得られた．術後，肺の拡張は徐々に改善し，術後6ヵ月の胸部X線写真でほぼ完全に拡張した．極めて稀な症例と思われ，文献的考察を加え報告する．
