Abstract

Because of a large variation in plasma levels of neuroleptics in patients receiving the same dose, antipsychotics are candidates for plasma concentration monitoring. On the basis of several years of experience with therapeutic drug monitoring (TDM) of neuroleptics the requirements for cost-effective TDM are summarized. The TDM center must be involved both in the interpretation of drug concentration results and in the sampling strategy. Otherwise, TDM will have no effect. The demands on analytical methods are summarized. Direct benefits of TDM such as concentration adjustment of neuroleptics in plasma and indirect benefits such as compliance control and detection of drug interaction and nonresponders are discussed.
