Abstract

A 75-year-old woman with a history of breast cancer and use of intravenous zoledronic acid arrived at the dental office reporting upper jaw discomfort. On intraoral examination, bilateral bone exposures compatible with osteonecrosis were observed. Initially, the treatment consisted of oral antibiotic therapy (clindamycin 300 g) for 7 days, mouthwash with 0.12% chlorhexidine digluconate every 12 hours for 7 days and necrotic bone curettage. Antimicrobial photodynamic therapy (aPDT) sessions were performed with a Hand Duo Laser device, which has a continuous operation mode, a semiconductor light-emitting laser, a fixed power of 100 mW, and a spot size of 0.03 cm<sup>2</sup>. The red wavelength was used for 90 seconds. 0.01% methylene blue gel was used as a photosensitizing agent. The preirradiation time was set at 5 minutes. The interval between each aPDT session was 48 hours, and after 12 sessions complete regression of the lesion was noted.
