Abstract

Every physician must sometimes manage pain associated with malignant disease. Despite the prognosis, the problem of pain deserves an intelligent appraisal and a systematic plan for relief to conserve the patient's physical, mental, and moral resources and social usefulness as long as possible. Keeping in mind that the major objective in treating the pain of advanced cancer is to keep the patient both free of pain and fully alert, the selection of a method of tumor therapy from an array of alternatives demands study of the individual patient and careful consieration of the appropriate measures, the possibilities for success, and the limitations, benefits, and risks involved. With comprehensive attention to the physical and personal needs of the patient and family, the cancer patient's pain can be managed within a total framework of care. The preliminary findings of the more innovative approaches to pain control such as “patient-controlled analgesia” 60 using the latest in medical technology for improved delivery systems have now allowed us to optimize the agents we have available as well as to better understand the complexities of controlling cancer pain.
