Abstract
症例は52歳の男性で，胃部不快感にて上部消化管内視鏡検査をうけ，胃体部に多発性ポリープと噴門部に3型腫瘍，幽門前庭部に1型腫瘍を認め，いずれも中分化腺癌と診断された．他に消化管ポリポーシスは認めず，遺伝性疾患の家族歴や粘膜皮膚の色素沈着を認めなかったため，不全型Peutz-Jeghers（以下，PJと略記）症候群における多発胃ポリープにともなう多発胃癌と診断し，胃全摘術を施行した．病理組織学的診断で，胃PJ型ポリープを背景とした深達度SSとSMの胃癌と診断された．不完全型PJ症候群におけるPJ型ポリープは単発性で空腸発生例が多くほとんどが良性である．胃での報告はこれまで11例あり，癌化の報告は4例であった．不完全型PJ症候群における胃PJ型ポリープの経時的変化や癌化の可能性が完全型PJ症候群と同様であるかは明らかでなく，その治療や経過観察の方法について今後のエビデンスの蓄積が必要である．
