Abstract

A 56-year-old man who underwent aortic and mitral valve replacement for endocarditis 13 years prior came to us with dyspnea on exertion. Systolic and diastolic murmurs were significant, and transthoracic echocardiography revealed severe aortic and mitral regurgitation with left ventricular dysfunction. Although transesophageal echocardiography identified periprosthetic leakage at the mitral position, it was not clear whether a regurgitant jet was coming through the periprosthetic tissue (ie, periprosthetic leakage) or the prosthesis itself at the aortic position because of artifacts caused by the two mechanical valves.
