In contrast to the American College of Cardiology/American Heart Association guidelines [3], the College's position paper was written by two physicians, one of whom may have been biased by his own risk index. Another possible problem is that the College's guidelines do not include use of exercise capacity in risk assessment, despite the fairly widespread use of this measure.
