Outcome at mid- and long-term oncological follow-up is still debated. The aim of the study was to identify predictor factors of recurrence at mid-term oncological follow-up. METHODS An observational prospective longitudinal study was carried out at a University centre from June 2010 to February 2014. Patients with first diagnosis or with primary recurrence of clinically non-muscle invasive bladder cancer (NMIBC), having tumours of = 3 cm and = 4 lesions were enrolled. All the patients underwent electrical ERBT by a Collins loop. The primary study end-point was recurrence-free survival. Secondary outcomes were: feasibility, safety, the presence of detrusor muscle (DM) and the recurrence rate at the first follow-up (3 months) cystoscopy (RRFF-C). Statistical analysis was complemented with multivariable analysis for identification of risk factor for recurrence at mid-term-follow-up. RESULTS Of 87 enrolled patients, 2 showed a non-urothelial carcinoma and eleven showed muscle invasive bladder carcinoma at the definitive pathology. The study cohort consisted of 74 TCC NMIBC cases, mean age 71yrs ± 8, presenting with a mean tumour diameter of 2.05 ± 0.63 cm and a median number of resected tumours per patients of 1 (range 1-4). The two-year recurrence-free survival was 85.59%. All the ERBT samples showed the presence of DM and the RRFF-C was 5.4% (4/74). An extraperitoneal bladder perforation occurred in only one patient. At multivariable analysis, performed to assess the impact of possible prognostic factors on recurrence-free survival, only gender (male) and the presence of CIS were independent predictors of recurrence (table 1). When patients were stratified according to grade, gender and > 1 recurrence/year were independent predictors of recurrence only for patients with NMIBC-HG. CONCLUSIONS Our findings suggest that the presence of CIS and male gender might represent the most important risk factor for recurrence after two years of follow-up.
