Abstract

Green Cross Health is a relative newcomer to the New Zealand general practice environment. We have a clear mission to improve the patient experience and understanding of the health system and to develop new models of care that will support this set of values. We are in the process of implementing: 1. Risk segmentation of population to facilitate best use of services 2. a coach/apprentice model of care for prevention and the management of long term conditions. 3. Integrated approach to Social and Health care service delivery to high needs populations. 4. Understanding how precision health may be applied within the primary care teams 5. Supporting Neighbourhood home initiatves in Northland and Hawkes Bay The presentation will address our progress in these areas
