Biochemical parameters including serum calcium, phosphorus, alkaline phosphatase (ALP) and PTH assay help in diagnosing PHPT. Ectopic parathyroid adenoma is a rare cause of PHPT. High index of clinical suspicion and a systematic approach is required for its diagnosis and management .Pathological fractures due to ectopic parathyroid adenoma is even rare .We present a case of pathological femoral neck fracture as a result of PHPT due to ectopic parathyroid adenoma situated adjacent to the Aoroto pulmonary window . Stabilization of the fracture once the PTH hormone levels have settled following treatment of hyperparathyroidism are the mainstay in the treatment of this rare clinical entity.
