Abstract

Disagreeable behaviors frequently shown by patients with chronic illness are described as the patient's best efforts to cope with his stressful situation, and as representative of reactance behaviors that are aimed to re-establish control and maintain the patient's freedom. Paradoxial interventions based on the expectation of patient compliance or patient defiance are discussed, and a rationale for planning the type of interventions is presented. Long-standing problems are described as chronically mishandled, but as offering as much opportunity for change as acute problems.
