Abstract

A 33-year-old male presented with recurrent palpitations and documented regular narrow complex tachycardia. Pre-excitation suggestive of a right sided accessory pathway was seen during sinus rhythm. During the electrophysiology study, a wide QRS tachycardia with a right bundle branch block (RBBB) morphology and earliest atrial activation in the His region was induced with a single atrial extrastimulus.
