Abstract

In the article by Avitabile and associates, 1 Avitabile T. Longo A. Reibaldi A. Intravitreal triamcinolone compared with macular laser grid photocoagulation for the treatment of cystoid macular edema. Am J Ophthalmol. 2005; 140: 695-702 Abstract Full Text Full Text PDF PubMed Scopus (141) Google Scholar they compared the efficacy of intravitreal injection of triamcinolone acetonide (TA) with macular laser grid photocoagulation for diabetic cystoid macular edema. However, they did not refer to the procedure of paracentesis. There are some points that I would like to add in this regard. ReplyAmerican Journal of OphthalmologyVol. 141Issue 5PreviewDr Lin and associates suggests in routine execution of a paracentesis before (or after) the intravitreal triamcinolone acetonide (IVTA) injection to prevent great IOP increase immediately after the injection. Intraocular injection of gas or fluid can cause an IOP increase that can lead to the occlusion of central retinal artery. Control of retinal perfusion by ophthalmoscopy is mandatory, and if the artery should remain closed for five to 10 minutes, paracentesis is indicated.1 After the injection of 0.3 ml of C3F8, occlusion of the central retinal artery was reported in some cases,1,2 but paracentesis was indicated in few cases.
