Abstract

In this article we report a case of papillary cystadenocarcinoma (PCAC) occurring in the upper lip. The lesion was located on the central region in the upper lip of a 92-year-old female. A borderless faintly opaque image was observed on X-rays, but no significant finding was found on MRI. On gross examination, the tumor was cystic overall.Histopathologically, a cyst-like structure containing mucous material in the cystic lumen was observed in the submucosal region. The lesion comprised multi-layered cells with tall columnar and mucinous characteristics that grew in a papillary configuration protruding into the cystic lumen. Cellular atypism, including abnormal mitotic figures and nuclear hyperchromatism, was evident, and the focus of the tumor nest infiltration was found in the neighboring region. Immunohistochemical data revealed positive staining for PCNA and Ki-67 which indicated proliferative activity, and also positive staining for p53 and CEA which indicated the nature of malignancy. We also briefly reviewed the previously reported cases of PCAC in the minor salivary glands published in the English literature, which consisted of 28 cases os such lesions including the present case. The statistical analysis provides insight into the low-grade malignant nature of the present tumor.
