Abstract

Weakness of trunk muscles affects sitting position negatively and worsens upper extremity movements in Spinal Muscular Atrophy (SMA) Type 2 patients. There are studies about postural muscle activities, equilibrium reactions, and upper extremity function differences with sitting position adaptations; but many of them are about children with cerebral palsy. Our aim was to see the differences in upper extremity functions with trunk support in sitting position. Upper extremity functions of five years old boy diagnosed with SMA Type 2 was evaluated after implementing sitting support (STABÝLO CONFORTABLE,NOEL, Poland) which is adapted for boy’s trunk with negative pressure pump to harden enough to support trunk. Upper extremity endurance (elbow flexion and extension in 30 s) and active shoulder flexion (with digital gonyometer) were measured while with and without sitting support. Active shoulder flexion degrees in sitting without support were 121 in right side and 123 in left side. After positioning boy with sitting support, these values increased to 137 in right side and 136 in left side. While sitting without support, shoulder flexion and extansion number was 19 but after sitting with support it increased to 29. It was shown that trunk support affects upper extremity performance and endurance in SMA Type 2 patient. This type of trunk support can provide proper postural alignment for upper extremity performance and prolonging daily activities. For this reason it must be considered in rehabilitation programs.
