Abstract

To describe four cases of cervical ectopic pregnancy after embryo transfer managed successfully with ultrasound-guided aspiration with single dose intraamniotic installation of methotrexate (MTX). Case report. From Jan 2005 to April 2006, four women with cervical ectopic pregnancy after embryo transfer were found in our unit. Under transvaginal ultrasound guidance, puncture, aspiration and injection of MTX(50mg/m2) was performed. All cervical pregnancies were diagnosed by transvaginal ultrasonography and fetal activity was found in two cases. The range of patients’age was 30-34 years old. The range of hCG level before treatment was 4826 IU/L -122300 IU/L. The range of gestation was 7 weeks to 8 weeks. Four patients presented vaginal bleeding. The procedure lasted 3-10 minutes and no active hemorrhage was observed during and after surgery. Serum hCG recovery after treatment was about 30-60days. During first week after procedure hCG level increased in 2 cases, while decreased in 6 cases. Two cervical ectopic pregnancies were successfully aborted. The resolving of enlarged cervical mass needed 3-5months. Transvaginal ultrasound-guided aspiration with single- dose methotrexate (MTX) intraamniotic installation can be safely used to treat cervical ectopic pregnancies.
