Abstract

8%-10% of patients with advanced cancer have intracranial metastases. At present, the treatments mainly include surgery, SBRT, whole brain radiotherapy, chemotherapy et al, but once the patients cannot tolerate those methods, palliative care shown good effects. Plant-extracted drugs, which are important part of palliative treatment, play important roles in inhibiting tumor progress, reducing adverse reactions of radio-chemotherapy, and improving the quality of life. 54 cancer patients, who diagnosed in brain metastasis and were failure of multi-line therapy, were enrolled. The plant-extracts grugs (Xianhe Baijiang and Shenhuang capsules) were used, which contain Agrimonia pilosa Ledeb, Patrinia scabiosaefolia, ginseng and Astragalus et al. Intracranial site, clinical symptoms, median survival, KPS score, and side-effects were observed. The percentage of supratentorial metastases, subtentorial metastases, and both metastases were 88.88% (48 cases), 5.56% (3 cases), and 5.56% (3 cases), respectively. Since the diagnosis at the first time, dizziness, headache, and decreased muscle strength were observed in 25 patients (46.30%), and 29 patients (53.70%) were symptomless. After treatment, KPS score was 43.70±27.77 compared to 39.63±23.95 (p=0.028), which improved up to 51.85%. Meanwhile, according to CTCAE3.0, no level II or above drug-related digestive and hematology side-effects were observed. Last, the median survival (mOS) reached to 4.5 months since the patient received plant-extracted palliative care treatment, and the longest survival time was up to 102 months. In summary, large proportion of brain metastatic tumors occurred at supratentorial without intracranial symptoms, which led to miss early diagnosis and treatment opportunities. Also, the plant-extracted treatment to patients, who suffered in low KPS score and failure with multi-line therapy, could improve the quality of life and prolong the survival time.
