Abstract

To assess the improvement of ultrasonographic fetal imaging following transabdominal amnioinfusion to severe midtrimester oligohydramnios with intact membrane. Eight pregnant women referred for unexplained midtrimester oligohydramnios were retrospectively reviewed. Videotapes of those undergoing diagnostic antenatal amnioinfusion were analyzed for quality of visualization of routinely imaged structures before and after the infusion procedure. After amnioinfusion, the improvement of ultrasonographic fetal imaging was 31.1% (before-64.1%, after-95.2%, P = 0.08 Sign test). We were able to know information about fetal gastrointestinal tract and urinary system. All pregnancies were terminated due to detection of fatal structural anomaly of the fetuses except one exposed to ACE inhibitor. We autopsied all fetuses. Definitive diagnosis were bilateral multicystic dysplastic kidney (2 cases), bilateral polycystic dysplastic kidney (2 cases) and bilateral renal agenesis (3 cases). The other one was a fetus who was exposed to ACE inhibitor from early pregnancy, however the fetal structures were normal. Transabdominal amnioinfusion can facilitate the ultrasonographic diagnostic accuracy in severe midtrimester oligohydramnios with intact membrane.
