Abstract

Introduction: It is well known that children with hypothyroidism have abnormal lower gastrointestinal motility principally chronic and severe constipation. There is no reports about upper gastrointestinal motility in children, on esophageal motility. These abnormalities are usually reversibles with an adequate thyroid hormone treatment Methods: Prospective, longitudinal study of 10 children with CH studied at the Gastroenterology and Nutrition department of the Instituto Nacional de Pediatria between April and December 2004. The diagnosis of CH was done by Endocrinology department.We performed esophageal manometry before and after 3 months treatment with thyroid hormones.The manometry was done with solid state catheter microtransducer using Smart lab motility, system Sandhill Co.The catheter was French 7 with three sensors from Drager Co.Information was procesed with a Smart Graph 40801; 3.22 Sandhill Scientific. We studied the basal pressure of the lower esophageal sphincter (LES),percent of the LES relaxation, amplitude, duration and velocity of transmision of the esophageal waves. Parent consent was obtained in all cases. Protocol was aproved by Research and Ethic committees. Results: Median age was 35 days (9–60)8 female. The LES pressure was 19.3 ± 17.5 mmHg(15–69) and after treatment 16.48 ± 13.67mmHg (11–21.6) Esophageal velocity was 1.7 ± 0.65 cm/sec and after 2.14 + 0.76 cm/sec. There was no important changes in esophageal waves and LES relaxation. Only one patient 54 days old at the time of the first study showed abnormal esophageal motility, 60 % of the esophagus waves were no propagatives and it was no reversible after 3 months treatment. Conclusion: Children with CH have an abnormal esophageal motility.LES pressure improvement after 3 months of treatment as well as the esophageal motility.
