Intravenous drug use (IVDU) is a risk factor representing 29% of infective endocarditis (IE) cases in America. The tricuspid valve is more commonly involved than left sided valves in IVDU patients. We present a rare case of a right and left sided endocarditis from Serratia marcescens causing aortocavitary fistula (ACF).
