Abstract

Bilateral testicular torsion is a rare complication that may be seen after bilateral inguinal herniorrhaphy. This condition may cause testicular atrophy if it is not diagnosed in early postoperative period.In this study, we aimed to present a 41 year-old male patient with bilateral testicular atrophy as a result of bilateral testicular torsion after bilateral inguinal herniorrhaphy. 41 year-old male patient was admitted to our clinic with bilateral testicular pain, tenderness, and scrotal skin discoloration. The physical examination was compatible with testicular torsion. The patient had a history of bilateral inguinal herniorrhaphy operation 1 week before the admission. According to the testicular color doppler ultrasonography, there was no testicular vascularisation and there was bilateral testicular torsion. Bilateral testicular detorsion operation was performed for the patient. During the operation, partial testicular vascularisation was seen for both sides and testicular fixation was completed successfully.(Figure 1) On postoperative 1st month, bilateral testicular atrophy was detected according to the color doppler ultrasonography and there was defective vascularisation. Also there was serious loss of libido for the patient and according to the laboratory results, total testosterone level was 0.07 ng/ml, luteinising hormone level was 31.58 mIU/ml, follicle-stimulating hormone level was 48.57 mIU/ml. Intramuscular testosteronereplacement therapy was started for the patient and the patient was invited for routine follow-up.
