Abstract

IntroductionDoppler Perfusion Index (DPI) and other parameters for liver circulation has been used in the detection of colorectal metastases in liver. The examination of splanchnic arteries, which include the hepatic artery as well, provides new information on flow in parenchymal abdominal organs. MethodsThe study involved 20 healthy controls and 30 patients without and 17 patients with liver metastases of colon cancer. The patients were observed in respect of Dukes stage (A, B, C and D), and number of metastases. Investigation of DPI was performed using color Doppler B mod ultrasound (Toshiba Core Vision) with duplex ultrasound of 3.75 MHz. Color Doppler signal intensity. ResultsDPI is significantly different (ANOVA, p<0.05) in patients with presence of metastases (mean values 0.355, max values 0.58 and min values 0.26) in comparison to patients without metastases (mean values 0.28, max values 0.44 and min values 0.15) or healthy patients (mean values 0.2 max values 0.27 and min values 0.10). We found significant negative correlation between DPI and mean values of hepatic artery diameter in healthy persons and significant positive correlation (p<0.05, Pearson correlation) in patients with liver metastasis. ConclusionThe objective of our study was to analyze the hepatic circulation through specific hemodynamic parameter determination, flow rate of hepatic blood vessels. Based on these results we suggested estimation of DPI and other parameters as an inexpensive and non-invasive method to evaluate the hemodynamic changes of the arterial and venous blood for confirmation of liver metastasis in respect to presence of the colon cancer.
