Abstract

A 26-year-old woman at 15 weeks’ gestational age presented with abdominal pain. Ultrasonography showed a 17 cm × 13 cm × 22 cm clear cyst of uncertain origin that crossed the midline. A pelvic magnetic resonance imaging scan showed ovarian parenchyma compressed at the periphery with an edematous right adnexal vascular pedicle twisted along its axis (Fig. 1A and B). A cancer antigen–125 test was not obtained preoperatively owing to concern for acute torsion and the patient's gravid status. The patient was counseled, and she proceeded with ovarian detorsion and cystectomy.
