Abstract

Background: Total pancreatectomy with islet cell auto-transplant is an effective treatment for chronic pancreatitis and avoids the complication of brittle diabetes. The current standard of care for islet auto-transplant is intraportal infusion of processed islets. However, concern for portal vein thrombosis limits islet cell infusion volume. Previously studied alternatives included kidney, omentum, lung, and pancreatic bed. Our group previously suggested the preperitoneal space may be an appropriate location for injection as has been demonstrated in mouse models, although no human studies have been reported. Here we report our initial experience with islet cell auto-transplant into the preperitoneal space during total pancreatectomy for chronic pancreatitis.
