Abstract

Protruding ears are frequently the focus of social derision, particularly in the school-aged child. Otoplasty can be performed to correct this problem with resulting pleasure for both patients and their families. An otoplasty protocol is described, including the goals, principles, operative procedure, and postoperative patient care. Possible complications are also discussed. To maximize the goals of a normal looking ear, and ear-to-ear symmetry, a system of simple pre- and postoperative measurements, based on the Frankfort line, is explained. The “four Ds” of otoplasty, including diagnosis, documentation, determination of surgical goals, and doing the worse ear first, are emphasized. A recommended order of the operative procedure, particularly placement of the conchal set-back sutures prior to the antihelical correcting sutures, is presented and the rationale explained.
