Abstract
Bone sarcomas account for 7% of malignant tumors in children, and osteosarcomas make up 35–50% of them. Many patients are diagnosed at spread stages of the disease, which dictates the need to search for new approaches to treatment. At the same time, the optimal therapeutic strategy is unknown, the results of treatment of such children remain unsatisfactory. Part I of this article describes currently used treatment regimens. Part II presents achievements in the field of personalized therapy. Thus, changes in treatment approaches illustrate the vector of development of the treatment principles of such complex diseases, being fatal for most patients a few years ago.

Summary
Bone sarcomas account for 7% of malignant tumors in children, and osteosarcomas make up 35–50% of them. У пациентов с локализованным вариантом 5-летняя общая выживаемость не превышает 75%, 5-летняя бессобытийная* выживаемость – 62% Наибольший интерес в лечении детей с локализованным вариантом остеосаркомы представляют исследования Итальянской и Скандинавской групп (Italian and Scandinavian sarcoma group – ISG/SSGI, SSG XIV), Французской детской онкологической группы (Societe Francaise d’Oncologie Pediatrique – SFOP OS94), а также Европейской и Американской групп по изучению сарком (European and American Osteosarcoma Study group – EURAMOS). После удаления первичного опухолевого очага выраженный гистологический ответ (лечебный патоморфоз 3–4-й степени) достигнут у 63% пациентов, неблагоприятный гистологический ответ (лечебный патоморфоз 1–2-й степени) – у 37%.
