Abstract

Orthokeratinized odontogenic cyst (OOC) is an uncommon developmental cyst, which is distinct from an odontogenic keratocyst. The first case describes a 26-year-old Caucasian man, with no complaints, examined for orthodontic reasons. The second case describes a 37-year-old Caucasian man complaining of a swelling on his face since 2 weeks prior to presentation. Well-circumscribed, unilocular, radiolucent areas with sclerotic borders, associated with the maxillary right third molar and the mandibular right third molar, both unerupted, were observed on the panoramic radiographs of the first and second cases, respectively. The diagnostic hypotheses included dentigerous cyst, odontogenic keratocyst, unicystic ameloblastoma, and odontogenic myxoma. Excisional biopsy, followed by a microscopic analysis, revealed cystic cavities lined by orthokeratinized stratified squamous epithelium, as well as capsules of dense fibrous connective tissue. A diagnosis of OOC was achieved, and follow-up of both patients showed no recurrence over 2- and 13-year periods, respectively.
