Abstract

Abstract A severe abrupt headache occurring with exertion is suggestive of subarachnoid hemorrhage, other intracranial hemorrhage, or arterial dissection. Thus, it must be aggressively worked up. Recurring exertional or sex-induced headaches are usually more benign. Nevertheless, it is imperative to rule out serious causes of ANY sudden or “thunderclap headache.” In this chapter, the wide range of possible causes of thunderclap headache is discussed, including both primary and secondary causes. A parsimonious diagnostic path is outlined, aimed at quickly discovering dangerous causes of these headaches. Treatment options for orgasmic headache are also presented.
