Abstract

A 69-year-old man, on treatment for multiple myeloma, was referred to the dental division, complaining of pain and difficulty in chewing, owing to an overgrowth of soft tissue located in the maxillary tuberosity and alveolar ridge of the right maxilla. This lesion was firm to palpation and showed areas of ulceration and spontaneous bleeding. The patient was a chronic alcoholic and smoker and presented with a general malaise. An incisional biopsy was performed on an outpatient basis. Histopathological examination revealed a proliferation of plasmacytoid-like cells immediately below the mucosa. These cells were arranged in sheets, pleomorphism was evident, and the tumor stroma was richly vascularized. The immunophenotype demonstrated that these cells were lambda chain (+), plasma cell (+), kappa chain (−), CD20 (−) and LCA (−). The diagnosis of extramedullary plasmacytoma was rendered and the patient was referred to the hematology service for treatment.
