Abstract
A 64-year-old woman underwent uneventful cataract surgery with implantation of enVista IOL (Bausch + Lomb) preloaded single-piece hydrophobic acrylic intraocular lens (IOL) MX60PL (Fig A). At 1-day postoperatively, a central diagonal opacity of unknown origin was observed (Fig B). After replacement of the lens, significant marks were observed on the lens under standard microscopy (Fig C). After IOL-exchange surgery, the visual acuity improved from 20/30 to 20/20. The original lens was sent to a forensic laboratory for evaluation.
