Abstract

The malignant transformation of leukoplakias is an important issue, on the one hand because the lesions are often present for a long time and actching of the malignant transformation is no eassy, and on the other hand because they spread over a large area. Material and method Between 2006 and 2012 a number of 12 patients presenting malignant transformed leukoplakias were operated; seven were men and five women. In four of the patients, the leukoplakias were multiple, in four of them, the leukoplakias were located behind the corner of the mouth and on the buccal mucosa, in two of them on the border of the tongue, in one of the patients the lesion was located on the floor of the mouth, and in another patient on the gingiva. A modified radical neck dissection was performed at eight patients with clinical lymph node involvement. For the plasty of the defects, we used: two fascio-cutaneous temporo-parietal island flaps in two cases a fascial temporo-parietal flap, in another case a full thickness skin graft, while in the rest of the patients direct closure was performed. In four of the cases postoperative radiotherapy was performed, associated in one case with chemotherapy, while in one of the patients the chemotherapy alone was administered. Results There was no postoperative relapse of the tumor in any of the patients. The functional and aesthetic postoperative results were good and very good, and the quality of life of the patients very good. Conclusions The malignant transformation of leukoplakias has a good prognosis and allows a good quality of life, although in certain cases the plasty of the defects is difficult due to the large surface along with reduced depth of the defects resulted from the surgical excision of the tumors.
