Conclusions: Measuring the distance between the aorta and the trachea is easy to learn and takes less than 20 seconds to obtain, with easier recognition of the landmarks on the parasagittal view than the axial one. Its reliability in assessing the pouch sign in more than 10 000 fetuses is interesting, but the pouch sign is so difficult to visualize that it cannot be utilized as a first sign of pathology. Moreover, it occurs only in atresia without fistula of the upper part of the esophagus.
