Abstract

Objectives To analyze the treatment outcomes in advanced Hypopharyngeal squamous cell carcinoma treated with either surgery or with organ preservation approaches of chemoradiation. Design Retrospective study. Patients Patients with Stage III and IV hypopharyngeal carcinoma with histopathologically proven squamous cell carcinoma. Methods A retrospective study of 60 consecutive cases of advanced hypopharyngeal squamous cell carcinoma treated in our institute. The study period was from June 2005 to June 2011. Treatment outcomes in terms of five year overall survival and recurrence free survival and local control rates were analyzed. Kaplan Meier survival curves were used for analysis. Results About 51 (85%) case were males and 9 (15%) case were female. 42 patients had pyriform sinus involvement, 17 had post-cricoid involvement and one patients had posterior pharyngeal wall involvement. 34 cases (56.7%) were considered for organ preservation and 26 (43.3%) cases were treated by surgical modality. 20 (33.3%) cases had recurrence. The mean follow up period was 20.6 months (Range 1 to 70 months). 43 patients were alive and disease free at the end of the follow up period. The 5-year overall survival was 37.8% and 40.8% for organ preservation and surgically treated group respectively. Similarly 5-year recurrence free survival rate was 41.5% and 48.4% respectively. Conclusion An Organ preservation approach with chemoradiation is an equally effective treatment modality for advanced hypopharyngeal carcinoma as compared to the surgical modality.
