Abstract

Pharyngeal reconstruction in the irradiated neck is a major surgical challenge. Local flaps are only suitable for small defects in patients who have not been treated by radiotherapy (Conley, 1956). Distant flaps. such as the delto-pectoral, can provide both pharyngeal lining and cover but several stages are required (Bakamjian. 1965). Castro-intestinal segmenrs can be transferred as pedicled flaps using a pre-sternal or intra-thoracic approach or as 3 free microvascular transfer (Baudet PI trl.. 1978: Bakamjian and Calamel, 1977; May and Bunting, 1977: O’Brien, 1977 1. This paper describes a one-stage pharyngo-oesophageal reconstruction of a defect extending from the base cjf the tongue to the root of the neck.
