Abstract
Older adult patients are most likely to reside in the community as they age. The opportunity to provide quality comprehensive palliative nursing care that incorporates age-specific assessment and management principles in the care of older adults is necessary within community settings. This chapter specifies the key considerations for geriatric palliative care nursing care of older adult patients who live at home. A summary of geriatric palliative care nursing principles that emphasizes person-centered care, advocacy, and the incorporation of essential care strategies that support physical, psychological, social, and spiritual well-being of older adults is reviewed. This chapter provides an overview of the complexities of aging and the mechanisms necessary to allow individuals to age successfully until the end of life.
