Abstract

ture Care. The information generated from the database aims to change practice through a continuous audit cycle to raise standards of care for this patient group. In the UK there are many different models of care delivery for hip fracture patients. However, the preferred option as described within the Blue Book on fragility fracture care is that of interdisciplinary working between orthopaedics and orthogeriatrics with nurses co-ordinating care. The purpose of this presentation is to outline the successes of the NHFD and to describe how one inner city hospital has implemented an interdisciplinary model of care where nurses take a lead in coordination and the positive effect this has had on patient care.
