Abstract

Objective To observe the effect of non -invasive positive pressure ventilation (NIPPV) combined with allylnoroxymorphone on chronic obstructive pulmonary disease (COPD) with pulmonary encephalopathy.Methods 86 cases of COPD patients combined with pulmonary encephalopathy were randomly divided into 2 groups,treatment group and controi group. On the basis of routine treatment,the control group was treated with NIPPV,while the treatment group was treated with NIPPV combined with allylnoroxymorphone.Results The levels PCO2 was significantly improved at 2 and 48 hours in treatment group compared to those at baseline ( P <0.01).The level of PCO2 and pH at 2 and 48 hours in treatment group was significantly superior to those in control group ( P <0.05 or P <0.01).The tracheal intubation rate of the treatment group was 23%,while that of the control group was 42%.Conclusions Compared with NIPPV treatment alone on COPD patients with pulmonary encephalopathy,combined NIPPV and allylnoroxymorphone treatment can improve carbon dioxide retention and the level of pH more earlier and effective,and can shorten the time of consciousness recovered,decrease the tracheal intubation rate.
