Abstract
The Transitional Care Unit at Fairfield Medical Center initiated a Medication Pass Supplement Program in late December 1996 in response to concerns over residents experiencing unacceptable weight loss during their stay. This program is currently ongoing and targets specific residents possessing established nutritional risk factors. The program involves the nursing staff administration of a dietitian-reviewed and approved nutritional supplement with each oral medication pass for residents meeting the criteria for program participation. Eleven months Of data have been collected and program results to date are very encouraging. The authors selected two case studies involving program participants which assist in showing the success of the program to date.
