Abstract

This article reviews a variety of nutrition assessment screening tools and protocols that have been developed for use in the community setting. The use of protocols and tools can help to identify patients/clients who may be malnourished or at risk of developing malnutrition. Such patients/clients can then be referred, as appropriate, to the dietitian or other members of the multidisciplinary team for a more detailed and comprehensive assessment. Before using screening tools in the practice setting acceptable standards for validity, reliability and other measures of accuracy should be met. Early identification of at-risk individuals combined with effective nutritional support may reduce hospital admissions and duration of stay, improve recovery from illness and attenuate morbidity and mortality associated with malnutrition.
